MEDIA FESTIVAL ENTRY ($75.00 PER SUBMISSION):
A $75 CAD administrative fee will be charged for each submission to the 7th International Symposium on Pediatric Pain Media Festival. All participants will need to officially register for the ISPP. When completing the registration form, please indicate the total number of submissions.

  • Payment can be made by Cheque, Visa, MasterCard, Money Order, Bank Draft or Bank Transfer. All payments must be made in Canadian Currency.
  • Charges on your credit card statement will appear as “International Conference Services” and will be converted to your currency.
  • Please ensure that your name and the words “EVENT: 7th INTERNATIONAL SYMPOSIUM ON PEDIATRIC PAIN” appear clearly on money orders, bank drafts, bank transfers or cheque payments.
  • Make cheques payable to “International Conference Services.”
  • Bank Transfers to: Transit # 0040, Account # 1710-551, SWIFT Code #BOFMCAM2.
    Bank Address: Bank of Montreal, Main Branch, First Tower, 595 Burrard Street, Vancouver, BC, V7X 1L7.
  • Bank drafts, money orders and cheques will only be accepted in CAD dollars and must be drawn on a Canadian Bank.
  • It is the delegate’s responsibility to ensure all bank transfer fees are paid over and above the registration fees owed. Otherwise, delegates will be asked to pay any outstanding balance at the on-site registration desk.
  • Please note all prices are in Canadian dollars. Payments made over the internet are via a secured site.

*With the registration form, Trainees MUST state the name of the Institution
where they are studying and working. As well, submit an official letter or
photocopy of their 2005/2006 Student ID from the Institution where they
are studying and working, indicating proof of their trainee status. Please
mail or fax with the Registration Form to +1 604 681 1049. Registration will
not be processed without receipt of this documentation.

** Developing & Currency Restricted Nation fees only apply when payment
of fees comes directly from the individual or institution registering AND the
individual attending is not sponsored by a third party company or
organization. A letter stating the reasons why the individual should be
considered eligible for the fee must accompany Developing Nation
registrations. Your registration will not be considered complete without a
letter providing required information, as detailed on the ISPP2006 website.

All registrations must be accompanied by payment; otherwise, you will not be considered registered.

 

REGISTRATION CONFIRMATION
All registrations will be acknowledged in writing with confirmation of delegate requirements according to the submitted Registration Form and upon receipt of full payment. The Registration Form attached acts as an Invoice.


REGISTRATION CANCELLATION & REFUND POLICY
Registration cancellations received in writing at the 7th International Symposium on Pediatric Pain Secretariat’s address by May 1, 2006 will be accepted and all fees refunded, less a CAD$100 administrative fee (as per Committee policy). Cancellations received after May 1, 2006 will not be accepted or refunded. However, transfer of your registration to another person is acceptable. A completed Registration Form for the new person must be faxed or mailed to the Symposium Secretariat prior to the Symposium, explaining who is being replaced. DO NOT USE THE ONLINE
REGISTRATION FORM FOR THIS PROCEDURE.
No refunds will be made for non-attendance at the Symposium.

In the unlikely event that the 7th International Symposium on Pediatric Pain is cancelled, travel and accommodation costs will not be refunded.
